Office Policies

Before scheduling below, review the required office forms/policies. By scheduling an appointment below, you are attesting to having read, reviewed, and agreeing to our office policies. If you have any concerns about our office policies, you should NOT schedule an appointment. We only evaluate patients 18 years of age and older. Appointments should only be scheduled by the patient or legal guardian if applicable. Patients may bring one ADULT guest.

Cosmetic / Self-Pay Pricing

Cosmetic evaluations are not covered by medical insurance. The new patient fee is $255 and return visits are $195. There is a surcharge for procedures. Additional information regarding self-pay pricing can be found here.

Medical Insurance

At this time, we are not accepting new patients with medical insurance coverage. Our practice has exceeded capacity and we are exploring ways to decrease the wait time for appointments. Dr. Mettu continues to care for established patients with insurance and is currently accepting new and established cosmetic patients on a self-pay basis.

Reason For Visit

Our practice is focused on a core set of procedures (within the field of oculoplastic surgery) that we perform regularly. Before scheduling, please review the list of procedures/conditions to ensure your condition is one that we evaluate/treat in our office.

When entering the 'Visit Reason' field either by web-based or text based scheduling, please be as specific as possible.

Confirmation

As your appointment nears, you will get confirmation texts from our Virtual Assistant to confirm your appointment. The text will come from 984-279-0313. If you do not confirm at least 24 hours prior to the appointment, your appointment may be canceled.

Medical History

Please be aware that we cannot offer surgery for patients who have pacemakers or defibrillators or who have lung problems requiring home oxygen use. If you have questions about if your medical history may cause problems for treatment/surgery. Please contact us before scheduling.

Late Cancelations/No Shows

Cancelations with less than 24 hours notice are subject to a fee of $50. No shows are subject to a fee of $100.
